Lower respiratory tract infections : impact on the workplace.

BACKGROUND: While there is some literature on the cost of specific respiratory infections, much of the existing research focuses only on direct medical treatment costs and does not take into consideration workplace burden due to disability and absenteeism. OBJECTIVE: To evaluate the impact of lower respiratory tract infections (LRTIs) on the workplace, specifically regarding the economic burden from an employer's perspective. DESIGN: Specific LRTI considered here were acute bronchitis, acute exacerbations of chronic bronchitis and pneumonia. Data from medical, prescription drug and disability claims of a large, national company in the US were used. These data provide information on both the healthcare and work loss impacts of LRTI on this major self-insured employer. The annual per capita expenditures for persons with LRTI were determined for beneficiaries (including employees and dependants) of this employer by analysing all claims in 1997. Results were compared with those of a 10% random sample of beneficiaries in the employer's overall beneficiary population. RESULTS: In 1997, total per person expenditures and total medical service utilisation were higher among beneficiaries with LRTI than among beneficiaries in the overall beneficiary population sample. Annual per capita employer expenditures for beneficiaries with LRTI totalled 6,116 US dollars, compared with 2,368 US dollars (1997 values) for the average beneficiary in the overall beneficiary population sample. Employer payments for lost work time through disability and absenteeism accounted for 17% of these expenditures amongst beneficiaries with LRTI and for 35% amongst employees with LRTI. CONCLUSIONS: Annual per capita expenditures for beneficiaries with LRTI were almost three times that of the average beneficiary. We conclude that it is important to consider work loss in analyses of the cost of LRTI.

Emergence of a coronavirus infectious bronchitis virus mutant with a truncated 3b gene: functional characterization of the 3b protein in pathogenesis and replication.

The subgenomic RNA 3 of IBV has been shown to be a tricistronic mRNA, encoding three products in IBV-infected cells. To explore if the least expressed ORF, ORF 3b, which encodes a nonstructural protein, is evolutionarily conserved and functionally indispensable for viral propagation in cultured cells, the Beaudette strain of IBV was propagated in chicken embryonated eggs for three passages and then adapted to a monkey kidney cell line, Vero. The 3b gene of passage 3 in embryonated eggs and passages 7, 15, 20, 25, 30, 35 50, and 65 in Vero cells were amplified by reverse transcription-polymerase chain reaction and sequenced. The results showed that viral RNA extracted from passages 35, 50, and 65 contained a single A insertion in a 6A stretch of the 3b gene between nucleotides 24075 and 24080, whereas the early passages carried the normal 3b gene. This insertion resulted in a frameshift event and therefore, if expressed, a C-terminally truncated protein. We showed that the frameshifting product, cloned in a plasmid, was expressed in vitro and in cells transfected with the mutant construct. The normal product of the 3b gene is 64 amino acids long, whereas the frameshifting product is 34 amino acids long with only 17 homogeneous amino acid residues at the N-terminal half. Immunofluorescent studies revealed that the normal 3b protein was localized to the nucleus and the truncated product showed a "free" distribution pattern, indicating that the C-terminal portion of 3b was responsible for its nuclear localization. Comparison of the complete genome sequences (27.6 kb) of isolates p20c22 and p36c12 (from passages 20 and 36, respectively) revealed that p36c12 contains three amino acid substitutions, two in the 195-kDa protein (encoded by gene 1) and one in the S protein, in addition to the frameshifting 3b product. Further characterization of the two isolates demonstrated that p36c12 showed growth advantage over p20c22 in both Vero cells and chicken embryos and was more virulent in chicken embryos than p20c22. These results suggest that the 3b gene product is not essential for the replication of IBV.

Childhood wheezing syndromes and healthcare data.

There is convincing evidence that several distinct wheezing syndromes exist in childhood. The purpose of this research was to assess the potential of using healthcare utilization profiles to identify wheezing syndromes in children which are distinct from asthma. Using population-based healthcare administrative data, a cohort of children, aged 5-15 years, with bronchitis diagnoses from time of birth to 1995, but no physician diagnoses of asthma, was followed over the period January 1996-March 1998. In this follow-up period, 13% had subsequent healthcare utilization for asthma, 23% had continued healthcare utilization for bronchitis, and 64% had no further healthcare utilization. The likelihood of bronchitis vs. asthma outcomes was determined for a variety of asthma risk factors. In a cohort of 11,043 children with initial healthcare contact for bronchitis but not asthma, two potentially distinct entities of bronchitis emerged from our data: 1) transient bronchitis, similar to transient wheezing of early childhood, which was associated with winter-only healthcare utilization and absence of allergy, and 2) recurrent bronchitis which differed from asthma on the basis of winter-only healthcare utilization, prematurity at birth, absence of allergy, and low socioeconomic status. Healthcare administrative records can be used to describe the natural history of wheezing in children and to identify markers which may discriminate asthma from other syndromes. 2003 Wiley-Liss, Inc.

Diseases of free-ranging chickens in the Qwa-Qwa District of the northeastern Free State province of South Africa.

A total of 177 free-ranging chickens from 19 Qwa-Qwa villages were bled from wing veins over a period of 6 months (June-November 2000). Serological tests indicated that 5% of chickens tested had been exposed to Newcastle disease, 43% to infectious bronchitis and 63% to Mycoplasma gallisepticum infection. McMaster and Visser sieve techniques were used to determine helminth and coccidia from pooled fresh faecal samples. Helminths isolated in 37% of the villages investigated were Heterakis, Ascaridia and Capillaria species. Eimeria species were also isolated in 32% of the villages investigated. The red fowl mite (Dermanyssus gallinae) was isolated from some of the birds and their nests. Data from a questionnaire survey indicated that all farmers interviewed had never received any technical support and that their chickens had never been vaccinated against any avian diseases. Only 10.5% of the owners interviewed had scientific knowledge on poultry diseases. There is an urgent need for the government to support free-ranging poultry farmers by providing subsidised vaccinations and technical support in order to develop and stimulate economic development in impoverished rural areas of South Africa.

Hair growth is a sophisticated biological process, which is still not thoroughly understood. However, due to the diversity of the problems underlying hair loss, there is no single solution for all hair loss cases. Most of chemical drugs and hair transplantation surgeries are not free from varying degrees of undesirable side effects on health.
Hair Million is an alternative solution to cope with hair loss problems. Anecdotally, it shows prositive results and improvement especially for age-related hair thinning and hair loss for a fraction of people who take it. We do not know the mechanisms of action as to how Hair Million works to help stop hair loss, and promote hair growth.
We only know by anecdotal observations. There has been no clinical trials nor placebo controlled statistical analysis on the efficacy of Hair Million on hair loss and hair growth.
