[Biochemical and immunological markers of osteoporosis in patients with chronic obstructive pulmonary disease]

A cytokine profile and changes in bone metabolism were studied in 95 patients (30 females and 65 males) with chronic obstructive pulmonary disease (COPD). Subnormal bone density was diagnosed in 78% patients with COPD. Frequency of osteoporosis discovery in emphysema was higher than in chronic obstructive bronchitis and the degree of density loss in bone tissue depended on ventilation disturbance. Intensification of bone resorption in relatively stable osteogenesis was encountered in 73.6% patients and manifested in elevated CrossLaps concentration. A close correlation exists between proinflammatory cytokines, bone density and indices of bone metabolism in COPD. This suggests a significant role of cytokine-mediated mechanisms in pathogenesis of pulmonogenic osteopenia.

[Domiciliary assisted ventilation in children]

The concept of assisted ventilation in the home setting has greatly progressed as a routine practice. This technique was used from 1990 to 2000 in 16 children cared for at our center. Ten children had neuromuscular disease (infantile spinal amyotrophy 6, Duchenne myopathy 3, and mitochondrial myopathy 1) or other conditions including central hypoventilation (n = 2), traumatic tetraplegia (n = 2), encephalopathy with chronic bronchitis (n = 1) and bronchopulmonary dysplasia (n = 1). Only 5 children had a tracheotomy, the others were treated successfully with non-invasive ventilatory assistance. Initialization of non-invasive ventilatory assistance had been planned before development of respiratory failure in 4 of the 11 children, but generally was indicated after an episode of acute respiratory distress. Home ventilation, particularly with non-invasive assistance is a reliable method for long-term treatment of chronic alveolar hypoventilation in children. The appropriate time for initiating this therapy should be better defined.

[Reported morbidity and use of health services by working women, Brazil]

OBJECTIVE: Inclusion in the work force has become part of women's daily lives, and they have to cope with multiple tasks and manage double working shifts. A study was carried out to assess working women's morbidity and their use of health services. METHODS: By means of a household survey, chronic and acute morbidity (i.e. within 30 days of data collection) was reported by 1,157 women aged 10 to 49 years living in the southern area of the city of Sao Paulo, Brazil. Data were collected from January 1992 to January 1993. The relationship between employment and marital status, educational level and socioeconomic status was evaluated. RESULTS: Women who had a paid job were more likely to complain of acute or chronic morbidity. Also, these women tended to be less likely to seek medical services for their health problem. The most prevalent acute illnesses were: respiratory illnesses, especially influenza or colds (72/1,000), abdominal and pelvic pain (13.2/1,000), headaches (11.4/1,000), and hypertension (9.5/1,000). Regarding chronic complaints, hypertension (39.7/1,000), back pain (26.5/1,000), gastritis or inflammation of duodenum (24.6/1,000), and bronchitis (24.6/1,000) were the most prevalent conditions. CONCLUSIONS: Although employed women reported more health complaints than women who were not employed, their use of health services was equal to or less than that of women who were not employed for most health conditions studied.

'Avoidable' mortality among immigrants in Sweden.

BACKGROUND: The concept of studying 'avoidable' mortality as an indicator of the outcome of health care has been applied mainly in studies of time-trends and geographical and socio-economic variation. METHODS: In this study, indicators of 'avoidable' mortality among immigrants in Sweden have been studied. Comparisons of death rates among immigrants and those born in Sweden were made using a linkage of the Population Census and the Cause of Death Register, nationwide sources. RESULTS: For a group of health policy indicators, such as liver cirrhosis and malignant neoplasms of the trachea, bronchus and lung, death rates were about 40-100% higher among immigrants from other Nordic countries, Yugoslavia and Eastern Europe than among the Swedish born population. For causes of death considered amenable to medical care intervention there were, however, small differences. For some conditions, such as cerebrovascular disease, malignant neoplasms of colon and rectum, chronic bronchitis and emphysema, high death rates were found among immigrants from other Nordic countries. For immigrants from other countries, there were no high death rates based on medical care indicators when compared to the Swedish-born population. CONCLUSION: There were few indications of inequity reflected in the mortality outcome of medical care. The variation found in death rates from health policy indicators may reflect differences in smoking and alcohol habits.
