Identification of patients likely to benefit from biofeedback for outlet obstruction constipation.

BACKGROUND: Biofeedback for outlet obstruction constipation has a varying success rate. The aim of this study was to identify which patients are likely to respond to biofeedback. METHODS: Thirty patients with severe outlet obstruction constipation were treated by a specialist nurse using three or four sessions of visual and auditory feedback of anal sphincter pressures. All patients were assessed by evacuating proctography, whole-gut transit studies and anorectal physiology before treatment. RESULTS: Two patients did not complete the course of biofeedback. Nine patients improved. Before treatment these patients had predominantly normal anorectal physiology and were all able to open the anorectal angle at evacuating proctography. Nineteen patients did not improve, of whom only three had no measured abnormality other than inability to empty the rectum. Ten of these patients had abnormal anorectal physiology which may have been due to previous vaginal delivery. CONCLUSION: Biofeedback for outlet obstruction constipation is more likely to be successful in patients without evidence of severe pelvic floor damage.

[Antiarrhythmic treatment of ventricular ectropic arrhythmies with Disopyramide]

Disopyramide (D.) is a new antiarrhythmic agent, which is not related chemically to any of the known substances. Animal experiments have shown a close similarity to quinidine action. Side effects are mainly due to anticholinergic effects. Pharmacokinetic studies with radioactive labelled D. have demonstrated that 80 per cent are elimated via the kidneys and 15 per cent through the gut. Gastrointestinal reabsorption in 90 per cent. We have studied the antiarrhythmic properties of D. in ventricular ectopic arrhythmias in twenty male patients. There were 13 myocardial infarctions, 5 cardiomyopathies, one severe oartic regurgitation with prosthetic valve replacement, one case with VPB of unknown aetiology. In ten cases the influence of D. on ventricular excitation threshold in implanted pacemakers was studied. The effects were correlated with Disopyramide-plasma levels. D. was effective in suppressing VPB. It was successful in 67 per cent, in 30 per cent the effect was unsatisfactory. Pacemaker-threshold remained unaltered. Side effects included constipation. Two deaths were observed. Their relationship to the adminstration of D. is not definitely proved. Nevertheless should the drug because of its negative inotropic action be employed with caution in cases with congestive heart failure.

Drug control of common symptoms in the terminally ill patient.

St Christophers' Hospice near London is now internationally known as a special centre for the care of terminally ill patients. In these cases, the relief of symptoms is paramount, and prominent among those symptoms is pain. Such pain can almost always be relieved without euphoria or lessening of consciousness. More than 60% of patients admitted to St Christopher's complain of pain, and the scheme of management outlined below results in substantial or complete relief of pain in all of them. Addiction does not occur when control of the patient's pain is part of the pattern of total care. The author considers management of pain of varying severity, together with associated symptoms such as vomiting, anorexia, dry mouth and hiccup, dyspnoea, cough, anxiety and depression, insomnia, constipation and diarrhoea.
