Many patients with terminal cancer would prefer to receive care at home. Frequently, however, socio-economic or familial constraints make this impossible, although from the medical point of view there might be no objections, even when the late stage of the disease is associated with certain complications. However, the preconditions must first be defined and agreed when the patient and relatives are making the necessary preparations--as far as possible jointly with a team comprising family doctor, oncologist and nursing service. Binding agreements among all those involved are required to ensure that in difficult situations the patient and his family receive the support they need. A number of common medical problems met with during palliative treatment, such as diarrhea, constipation, pleural effusion, etc. are described.

Posterior sagittal abdominoperineal pull-through: a new approach to definitive treatment of Hirschsprung's disease--initial experience.

PURPOSE: Posterior sagittal abdominoperineal pull-through (PSAPP) was applied for the surgical treatment of Hirschsprung's disease (HD) to decrease the incidence or eliminate the complications related to the major pullthrough procedures. METHODS: Ten children with HD underwent the new surgical procedure, PSAPR The diagnosis of the disease was established under 1 year of age in 7 children and between 2 and 5 years of age in three instances. The length of resected aganglionic segment ranged from 6 to 20 cm (mean, 11.9 cm). The follow-up period was 18 months to 4 years (mean, 2.7 years). Postoperative clinical and manometric results were compared with children after Rehbein procedure and with controls. RESULTS: Postoperative complications were observed in patients operated on first: anastomotic stenosis in three children, enterocolitis in three, constipation in two, anastomotic leak (iatrogenic injury) in one, and dehiscence of colostomy anastomosis in two patients. Improved surgical technique allowed to eliminate the causes of complications, and the last five children of this group who underwent surgery have had an uneventful postoperative outcome. CONCLUSION: The posterior sagittal approach with the posterior anorectotomy provides an excellent exposure of the operative field, allowing to perform the lowest possible resection and subsequent anastomosis.

Irritable bowel syndrome: clinical and psychopathological correlations.

Irritable bowel syndrome is a common disorder in gastroenterology consultations. Our objectives were to assess patients' psychic functioning, the relationships between psychopathology and pain, the degree of psychopathology in constipated and diarrheic subgroups, and if previous abdominal surgery was related to pain intensity. Forty-five gastroenterological outpatients, both sexes, were studied. Patients were grouped as predominantly constipated, predominantly diarrheic, or alternating constipation and diarrhea. After diagnosis, patients had: Psychologist half-guided interviews, Bender Visuomotor Gestalt Test, Hammer Graphic Tests, and Rorschach Psychodiagnostic Tests. The last one showed that 78% suffered from distortion in reality perception; 100% thought away from reality and had a deficit in organizing capability, synthesis, and integration. Within psychological gnosiology, these patients would be diagnosed as borderline personalities and this would imply an overlapping of neurotic and psychotic functioning. Affective vulnerability and overadjustment to environment were found, and this finding is an original one to the best of our knowledge.
