The MACE procedure: experience in the United Kingdom.

BACKGROUND/PURPOSE: The Malone Antegrade Continence enema (MACE) procedure has become commonplace in the treatment of patients with faecal incontinence and constipation. To ascertain its place in paediatric practice, the authors surveyed members of the British Association of Paediatric Surgeons (BAPS) to see if they have performed a MACE and continue to do so. The authors also assessed their indications, success rate, and complications. METHODS: Members were asked to give the diagnosis of the patients submitted for surgery before September 1996, the procedure used to constructthe MACE, the rate of failure, and complications encountered. RESULTS: A total of 300 MACE were reported, and the mean follow-up was 2.4 years. The diagnosis of 273 patients was known: spina bifida, 108; anorectal anomaly, 90; Hirschsprung's disease, 22; constipation, 23; other, 30. The most popular procedure was appendix disconnection and reimplantation. The overall success rate for all diagnoses was 79%. The main complication was stomal stenosis, 30%. CONCLUSIONS: These figures confirm that MACE is a useful operation. Improved patient selection, improvement in techniques, and increasing knowledge regarding continued management should lead to improving results across the country.

Colonic neuroendocrine peptide levels in patients with chronic idiopathic slow transit constipation.

The motility disorders in patients with slow-transit constipation have been attributed to a disturbance in the peptidergic innervation of the colonic enteric nervous system. The nature of this disturbance is, however, controversial. In the present study 7 patients with long-standing severe slow- transit constipation were included, and normal tissues from the colon of 6 patients, which had undergone colonectomy because of polyp, chronic diverticulitis, prolapsis and volvulus were used as controls. The concentrations of several neuroendocrine peptides were measured in tissue extracts by radioimmuno-assays. The level of pancreatic polypeptide was high in 2 patients and low in one patient. Peptide YY level was high in 3 patients and low in one patient, and that of neuropeptide Y was high in 4 patients. Somatostatin and vasoactive intestinal polypeptide levels were high in 3 patients and substance P concentration was low in 3 patients. Neurotensin level was high in one patient and low in another patient. Galanin concentration was low in 2 patients and high in one patient. Gastrin-releasing peptide level was high in one patient and that of enkephalin was high in 2 patients. All patients had altered concentrations of several neuroendocrine peptides except one, who had only a low level of galanin. It is concluded that patients with slow-transit constipation have disturbed neuroendocrine peptides in common, though the nature of this disturbance varies between patients and in most patients several neuroendocrine peptides were affected. This may explain the controversial results obtained in previous studies.

[A particular case of iodine-induced hypothyroidism]

A surplus of iodine taken through drugs or food can cause a functional thyroidal deficiency, especially if there are pre-existent alterations in the gland. The case described of a young woman with a iodine-induced hypothyroidism without goitre due to an excessive trans-cutaneous/trans-mucous absorption of iodine in sea-bath salts. The woman, aged 32, showed a symptomatology characterized by asthenia, ponderal increase, constipation and bradypsychia. The thyroid appears normal after echography and scintigraphy. Iodized sea-bath salts have been used for three months in a hot water bath for a slimming effect. A complete regression is noticed with their suspension after one year. An excessive supply of iodine can even if rarely cause a hypothyroidism with/without goitre, because of an intrinsic defect in the mechanism of the organic link which permits a persistent Wolff-Chaickoff effect. A genetic predisposition is supposed and also a higher frequency in patients with a preceding thyroiditis, Basedow disease or Hashimoto disease. Particular attention should be given to a differential diagnosis in case of hypothyroidism in young women with a normal thyroid and negative antibodies. It is clear that a careful pharmacological anamnesis is essential as well as a careful anamnesis on the way of life.
