Alopecia from oral contraceptives.

PIP: Diffuse alopecia of the scalp developed in 5 patients during or following the administration of oral contraceptives. Alopecia developing after discontinuance was comparable to postpartum telogen effluvium. That occurring during use (with either high dosage or prolonged administration) resembled alopecia of the malepattern type. Progestogens may be important etiologic agents in alopecia from oral contraceptives; their mode of action remains obscure but may be different from that in postpartum alopecia, since 3 patients observed no hair loss following pregnancies and 2 developed the condition with continued steroid administration. Alopecia from oral contraceptives may be a frequent cause of hair loss in women and may also be associated with other types of diffuse scalp hair loss.

Oral contraceptives: current status of therapy.

PIP: The Council of Drugs of the American Medical Association reviews the possible side effects of oral contraceptive use, including cases of predisposing factors. Nausea, anorexia, diarrhea or constipation, and changes in hepatic function are some gastrointestinal sympstoms. A relationships exists between contraceptive use and thromboembolic phenomena reflected by morbidity and mortality rates, which may be attributed to the high estrogen content of some preparations. Intravascular clot formation, changes in arterial and venous walls, cerebral arterial thrombosis, migraine, and systolic and diastolic hypertension have all been related to contraceptive use. Various effects on the endocrine system include delayed menses, changes, in carbohydrate metabolism, and increases in transcortin and thyroxinbinding globulin. Changes in the morphology of the cervix and in the endometrium have been noted. While carcinoma in situ has risen in incidence, it has been found that twice as many women on oral contraceptives have epithelial dysplasia, a precursor of cervical carcinoma, before therapy as those choosing other contraception. The association between breast or uterine carcinoma has not been determined. Other side effects include depression, tension, neuroophthalmic disorders, melasma, acne, and hair loss or hirsutism.

Neutrophil gelatinase-associated lipocalin is a marker for dysregulated keratinocyte differentiation in human skin.

Neutrophil gelatinase-associated lipocalin (NGAL) is a 25-kDa protein initially isolated from the specific granules of human neutrophils. It is a member of the highly heterogeneous lipocalin protein family, which shares a common tertiary structure. Its synthesis is induced in gastrointestinal epithelium in association with inflammation and malignancy. To gain insight into its potential role in other epithelia we have investigated the expression of NGAL in human skin embryonic development, in normal adult skin, and in skin associated with inflammation and neoplastic transformation. In the present study we report that the embryonic expression of NGAL appears to be regulated in a spatio-temporal pattern. It was induced in the interfollicular epidermis at 20-24 weeks of gestational age but thereafter progressively receded towards the hair follicles. In normal adult skin, NGAL was detected solely in association with hair follicles. However, strong induction of NGAL in the epidermis was seen in a variety of skin disorders characterized by dysregulated epithelial differentiation such as psoriasis, pityriasis rubra and squamous cell carcinoma. In these tissues production of NGAL was confined to spatially distinct subpopulations of keratinocytes underlying areas of parakeratosis, whereas skin samples lacking parakeratotic epithelium such as lichen ruber planus, acute contact eczema and basal cell carcinoma were negative for NGAL. Consistent with being a marker for disturbed terminal differentiation, NGAL immunoreactivity showed an inverse pattern when compared with that of the differentiation marker filaggrin. The biologic functions of NGAL in epithelia are not fully known, although an immunomodulatory role in host defense has been proposed. In addition, the transient interfollicular NGAL expression during skin embryogenesis along with the induction of NGAL in adult parakeratotic epidermis suggests it play a role in epithelial differentiation pathways.

[Good results with Norethisterone in rheumatoid arthritis]

PIP: The incidence of rheumatoid arthritis in women is 4 times that in men, suggesting a possible relationship between this disease and ovarian functioning. A study was done to attempt ovarian control in the treatment of rheumatoid polyarthritis by the use of norethisterone in a dose of 20 mg/day for 1 month. 3 women between the ages of 40 and 50 were helped greatly. In 5 women between the ages of 50 and 60 results obtained were quite good. 4 women over 60 had poor results. It is logical to suppose that results would be better in younger women. In 4 cases where metrorrhagia supervened and in 3 others treatment was not properly used. Some patients complained of loss of hair and in some there was a tendency to obesity and hypertension which, however, was generally mild. No case of rheumatoid arthritis was cured. Response, when it occurred, was rapid but once obtained was not followed by further improvement.

Hair growth is a sophisticated biological process, which is still not thoroughly understood. A multitude of therapeutic measures, including drugs, surgery, and suppelements have been made available, and used. However, due to the diversity of the problems underlying hair loss, there is no single solution for all hair loss cases.
