CASE REPORT: Two children were admitted for poisoning by organophosphate pesticides applied as hair rinses against lice. These chemical agents inhibit the acetylcholinesterase enzyme at various sites. The resultant accumulation of the transmitter acetylcholine causes abnormal signs and symptoms. The diagnosis is based on a reduction in the blood cholinesterase activity. The specific treatment comprises the administration of atropine and pralidoxime. CONCLUSION: Accidental poisoning by organophosphate insecticides may occur, due to the misuse of such substances as shampoo against lice. An accurate information for users is necessary.

Risk factors for epithelial erosions in laser in situ keratomileusis.

PURPOSE: To determine the risk factors for an epithelial defect during laser in situ keratomileusis (LASIK). SETTING: LASIK Vision, Toronto, Ontario, Canada. METHODS: In this prospective cohort study, 926 patients (1852 eyes) presenting for LASIK over a 6-month period were evaluated for age, sex, Fitzpatrick Skin Type (FST), eye color, hair color at 3 years of age, facial skin wrinkling, ethnicity (Lancer Ethnicity Scale [LES]), keratometry, Schirmer I reading, corneal thickness, and preexisting signs of corneal epithelial dystrophy. In all patients, LASIK was performed using the Technolas 217 laser (Bausch & Lomb), the Hansatome(R) microkeratome (Bausch & Lomb), and the same nomogram settings. RESULTS: Fourteen percent of patients had significant epithelial fragility. In patients with FST I or II or LES 1 or 2, the relative risk of an epithelial defect was 10 times greater than in other patients; in those older than 40 years, it was 6 times greater than in other patients; in those with lighter hair or eye color, it was 2 to 3 times greater than in patients with darker hair or eyes. There was no significant difference in pachymetry, vertical or horizontal keratometry, or Schirmer readings between eyes with epithelial defects and eyes without. CONCLUSIONS: The risk of epithelial erosions during LASIK strongly correlated with patients' skin type and age.

Ethidium bromide stimulated hyper laccase production from bird's nest fungus Cyathus bulleri.

AIMS: Effect of ethidium bromide, a DNA intercalating agent, on laccase production from Cyathus bulleri was studied. METHODS AND RESULTS: The bird's nest fungus, Cyathus bulleri was grown on 2% (w/v) malt extract agar (MEA) supplemented with 1.5 microg ml(-1) of the phenanthridine dye ethidium bromide (EtBr) for 7 d and when grown subsequently in malt extract broth (MEB), produced a 4.2-fold increase in laccase production as compared to the untreated fungus. The fungal cultures following a single EtBr treatment, when regrown on MEA devoid of EtBr, produced a sixfold increase in laccase in MEB. However, on subsequent culturing on MEA in the absence of EtBr, only a 2.5-fold increase in laccase production could be maintained. In another attempt, the initial EtBr-treated cultures, when subjected to a second EtBr treatment (1.5 microg ml(-1)) on MEA for 7 d, produced a 1.4-fold increase in laccase production in MEB. CONCLUSIONS: The white-rot fungus Cyathus bulleri, when treated with EtBr at a concentration of 1.5 microg ml(-1) and regrown on MEA devoid of EtBr, produced a sixfold increase in laccase production in MEB. SIGNIFICANCE AND THE IMPACT OF THE STUDY: The variable form of C. bulleri capable of hyper laccase production can improve the economic feasibility of environmentally benign processes involving use of fungal laccases in cosmetics (including hair dyes), food and beverages, clinical diagnostics, pulp and paper industry, industrial effluent treatment, animal biotechnology and biotransformations.

The aim of our study was to evaluate the age of the onset of pubertal characteristics in a population of Northern Italy and to compare our data with the pubertal staging scale defined by Tanner in 1976. These observations may help us to verify if even in Italy there is a positive secular change in sexual maturation, as has been observed in many European countries. We studied 3510 children (1868 males and 1642 females) aged 6-15 years, born between 1983 and 1996. In every subject sexual maturity was evaluated according to Tanner stages. We calculated the percentile distribution for ages of attainment of B2 and PH2 stages for females and G2, PH2 and testicular volume = or >4 ml (VT4) for males. In females the percentile values for B2 were: 3rd=8.1, 50th=10.5, and 97th=12.6 years. In males the percentile values for G2 and VT4 were: 3rd=8.8 and 9.1 years respectively, 50th=11 years, and 97th=13.4 and 13.3 years respectively. 3rd, 50th, and 97th centiles for pubic hair stage 2 were 8.1, 10.6 and 12.6 years for females and 8.8, 11.2 and 13.4 years for males. For all stages an earlier pubertal development was seen in comparison with Tanner data. One hundred-twenty-five girls had already reached menarche. Median (interquartile range) age at menarche was 11.9 (11.4-12.4) years showing a decrease compared with Tanner values. Our data clearly demonstrate a positive secular trend in pubertal development and point out the need of having updated references values of the timing of sexual maturation for the whole Italian population.
