Low serum levels of intact osteocalcin in patients with congestive heart failure.

Impaired bone metabolism is a frequent complication following heart transplantation. Little is known, however, about possible alterations in bone turnover of pretransplant patients with congestive heart failure (CHF). We therefore studied biomarkers of bone turnover in 21 male patients with CHF (New York Heart Association [NYHA] classification > II) compared with 21 controls (NYHA classification < II). Biomarkers of bone formation (intact osteocalcin and carboxy-terminal propeptide of type I collagen), markers of bone resorption (N-telopeptide and C-telopeptide of type I collagen), undercarboxylated osteocalcin (an indicator of fracture risk), and concentrations of calcium, parathyroid hormone, and vitamin D metabolites (25-hydroxyvitamin D and calcitriol) were measured in fasting blood samples. Serum levels of intact osteocalcin were 44.5% lower (P < 0.01), and the ratio of undercarboxylated-to-intact osteocalcin was 113% higher (P < 0.01) in the patients in comparison with the controls. Moreover, patients had 34% lower 25-hydroxyvitamin D levels (P < 0.01) and 22% lower calcitriol levels (P < 0.05) than the controls. The bone resorption markers, N-telopeptide and C-telopeptide; the bone formation marker, carboxy-terminal propeptide of type I collagen; parathyroid hormone levels; and albumin-adjusted serum calcium concentrations did not differ between patients and controls (all P values > 0.05). In summary, there were no biochemical signs of enhanced bone collagen resorption in pretransplant CHF patients. However, the low serum levels of intact osteocalcin and the high ratio of undercarboxylated-to-intact osteocalcin deserve further consideration.

First report on sequential totally endoscopic thymomectomy and adrenalectomy using computer-enhanced telemanipulation.

Background: Laparoscopic adrenalectomy is considered the standard method for removal of benign adrenal tumors, regardless of hormone activity. Minimally invasive surgery for thymomectomy aims at limited approaches, avoiding complete sternotomy or large thoracotomy. Methods: We report on a case in which totally endoscopic thymomectomy and adrenal gland resection were performed sequentially using a computer-enhanced telemanipulation system within 3 weeks. Results: Operating time was 4.5 h for totally endoscopic adrenalectomy and 1.5 h for totally endoscopic thymomectomy. The patient was transferred to the normal ward on the day of operation after either procedure and had an uneventful recovery. Pathology yielded no malignancy in both cases. Conclusion: This report demonstrates the safety and feasibility of various totally endoscopic procedures performed sequentially.

Intraoperative parathormone measurement in patients with primary hyperparathyroidism: a prospective clinical study.

BACKGROUND: The objective of this pilot study was to evaluate the use of intraoperative iPTH measurement in combination with bilateral neck exploration in patients with primary hyperparathyroidism. METHODS: A prospective study was conducted in a single centre university hospital in Switzerland comprising 33 patients with primary hyperparathyroidism undergoing neck surgery. No routine preoperative localisation studies were conducted. Intraoperative iPTH measurement was performed at defined times. Exploration was discontinued when iPTH fell below 15% of baseline. RESULTS: For all patients with adenoma the median serum iPTH decreased to 11.9% (standard deviation 5.5%) of baseline concentrations. All patients with hyperplasia achieved normocalcaemia. At median follow-up of 17.7 weeks neither recurrent nor persistent hypercalcaemia was observed. Eleven patients were explored unilaterally. CONCLUSION: Measurement of intraoperative serum iPTH is a helpful tool in the differentiation of single adenoma from hyperplastic glands. When we call for a strict 78% decrease in serum iPTH from base levels after 10 minutes, measurement of this marker protein even allows differentiation of single- from multi-glandular disease.

Low serum total thyroxine and free triiodothyronine in patients with hepatic encephalopathy due to non-alcoholic cirrhosis.

PRINCIPLES: We evaluated serum thyroid hormone levels in non-alcoholic cirrhotic patients with and without hepatic encephalopathy. METHODS: 15 consecutive patients with hepatic encephalopathy secondary to non-alcoholic cirrhosis (8 males and 7 females, age 37-75 years) and 33 non-alcoholic cirrhotic patients without encephalopathy (22 males and 11 females, age 36-74 years) were investigated. A control group consisted of 20 healthy subjects (10 men and 10 women aged 26-69 years). The levels of serum triiodothyronine (T3), thyroxine (T4), thyrotropin (TSH), free T3 (FT3) and free T4 (FT4) were studied in serum samples drawn in the morning. Thyroid function tests were set in relation to the severity of hepatic dysfunction and to the presence or absence of hepatic encephalopathy. RESULTS: Serum levels of FT3 and total T4 (but not total T3 and FT4) were significantly lower in patients with hepatic encephalopathy compared to decompensated cirrhotic patients without encephalopathy (p = 0.006 for T4, P <0.05 for FT3). Prothrombin-time also differed significantly between decompensated cirrhotic patients (Child C) with and without encephalopathy groups (p = 0.002). CONCLUSIONS: These results suggest that patients with hepatic encephalopathy secondary to decompensated non-alcoholic cirrhosis are typified by low FT3 and low total T4, as well as by a prolonged prothrombin time. Low FT3 does not obviously put patients at risk for hepatic encephalopathy, and thyroid parameters are secondary and late events.

A novel matrix-assisted laser desorption/ionisation quadrupole ion trap time-of-flight (MALDI QIT ToF) mass spectrometer has been used to analyse high mass peptide ions exceeding 2000 Da. Human adrenocorticotropic hormone (fragment 18-39) and oxidised bovine insulin chain B were utilised to evaluate the performance of the instrument both in MS and in MS/MS mode. Its ability to efficiently isolate ions and to fragment them using collisionally activated decomposition (CAD) has been demonstrated using mixtures diluted to the low-femtomole level on target. Additionally, multiple stage mass spectrometry (MS/MS/MS) provides a second-generation product ion spectrum in which new fragment ions are detected and new stretches of amino acids are identified. 2003 John Wiley & Sons, Ltd.
