Insulin-like growth factor 1 (IGF-1, somatomedin C) belongs to a family of polipeptide hormones, which are structurally close relatives of insulin. Circulating IGF-1 is synthesised in the liver. Serum level of somatomedin is regulated by: growth hormone (GH), insulin and nutrition. It is also produced locally by most tissues, where it acts in auto- and paracrine manner. IGF-1 takes part in regulating growth after binding to IGF receptor during embryonic development and after birth. In adults somatomedin plays a role in the process of regeneration, mainly in the case of connective tissue. It is also a weak mitogen for most cultured cells and it can act like insulin. Somatomedin circulates in plasma in complex with a family of binding proteins. 85-95% of total IGF-1 is found in the complex consisting of IGF-1, binding protein 3 and ALS. This complex is a store of IGF and limits the access of somatomedin to specific receptors. After binding with IGFBP-1, IGFBP-2 and IGFBP-6, IGF-1 passes through epithelium and reaches the target cells. The serum concentration of this protein appears to be inversely related to insulin level. IGFBP-1 can modulate IGF growth-promoting effect. IGF and its binding proteins are important in the diagnosis and treatment of some pituitary diseases, catabolic states such as malnutritoin, burns, AIDS, politrauma and tumors with hypoglikemia. Insulin-like growth factors may be involved in the etiopathogenesis of diabetes and in diabetes complications. Abnormalites in functioning of GH-IGF-1 axis are regarded as a cause of the growth retardation in children with poor metabolic control of type 1 diabetes, insulin-resistance, dawn phenomenon and fat disorders. rhIGF has been used in the treatment of some diseases bringing positive results.

The concentration of interleukin 6 (IL-6), soluble IL-6 receptor (sIL-6R) and intercellular adhesion molecule-1 (sICAM-1) was assayed in 24 children with Graves' disease, aged 6.5-17 years, before treatment with methimazole, during clinical and biochemical remission (8 weeks), and following the eighteen-month therapy with the thyreostatic. ELISA method was used to determine the concentration of interleukin 6, sIL-6R and sICAM-1. An increase was observed in serum concentration of IL-6 and its receptor sIL-6R and soluble intercellular adhesion molecule-1 in the course of Graves' disease. A significant reduction in sIL-6R (8 weeks) and IL-6 (18 months) was noted after therapy with methimazole. A positive correlation was found between microsomal antibodies and the concentration of sICAM-1 and sIL-6R, and between antithyreoglobulin antibodies and concentration of sIL-6R in children with fresh hyperthyroidism. No correlation was revealed between the level of thyroid hormones and the concentration of cytokines and adhesion molecules.

Hormones of the gastrointestinal tract are difficult to study because they are not produced by discrete groups of cells organized into glands. Instead, they are released from single cells scatterred along the digestive tract. Furthemore, as several of these peptides are found both in specific endocrine cells and in neurons and their nerve terminals it is difficult to establish which effects are dependent on peptide release from nerve terminals and which represent endocrine activity. The gastrointestinal peptides act locally on various organs and processes within the gastrointestinal system. There is a correlation between carbohydrate absorption, plasma concentration of glucose and release of gastrointestinal tract hormones.

[Pseudohypoparathyroidism in a 14-year old girl]

The authors describe the case of a 14-year old girl with pseudohypoparathyroidism, who showed characteristic phenotypic features of Albright's hereditary osteodystrophy (short stature, round face, mild obesity, abnormal position of teeth, lack of canines, limb valgity) and biochemical disturbances - hypocalcemia, hyperphosphatemia. The nature of pseudohypoparathyroidism consists in a lack of response to the parathormone of effector organs, such as kidneys, bones, alimentary tract. The treatment of choice includes active metabolites of vitamin D3, calcium salts, phosphate-binding components. Pseudohypoparathyroidism is a rare disease. Worth noting is the fact that it can occur in a child with height deficiency.

Struma caused by iodine deficit is an endemic disorder in specified regions. The purpose of the study was a biochemic characteristic of struma simplex in children from Bydgoszcz province and evalued correlation between laboratory exponents of thyroid gland function, size of the gland and kind of the struma. It was confirmed that neutral struma of children generally takes a course with euthyroidism. In children with struma were confirmed significant lower values of T4 and no significant higher values of T3. Value of TSH was no different in comparison with the control group. Proved was that hormonal thyroid activity is lower when struma is getting bigger.
