A 68-year-old man was admitted to our hospital because of muscle weakness. A complete medical examination led to a diagnosis of small cell lung carcinoma (SCLC) with Lambert-Eaton myasthenic syndrome (LEMS) and the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Four courses of chemotherapy (carboplatin + etoposide) and one of radiotherapy with a total dose of 45 Gy to the mediastinum were performed and resulted in a partial response in the SCLC. After the second course of chemotherapy, the serum level of antivoltage-gated Ca2+ channel (VGCC) antibody decreased from 190 pg/ml to 120 pg/ml. Marked improvement of the muscle weakness was recognized only after 3 courses of chemotherapy. The patient, who had had difficulty in standing, recovered enough to be able to climb stairs after 4 courses of chemotherapy. Marked improvement of LEMS was achieved by treatment for small cell lung carcinoma.

[Signet ring cell carcinoma of the prostate successfully treated with endocrine therapy: a case report]

A 67-year-old man complained of dysuria. The prostate was enlarged and stony hard. The serum level of prostate specific antigen was abnormally high (46.2 ng/ml). Prostatic biopsy showed signet ring cells which were stained positive for prostate specific antigen and poorly differentiated adenocarcinoma. Computed tomographic scan revealed the enlargement of the para-aortic lymph nodes. Endocrine therapy with luteinizing hormone-releasing hormone agonist was started. After two months, the serum level of prostate specific antigen decreased to the normal range. The pathological findings and the good response to endocrine therapy in our case suggest that signet ring cell carcinoma of the prostate is only a morphologic variant of ordinary prostatic adenocarcinoma.

It is well established that the obese gene product, leptin is an important circulating satiety factor that signals nutritional status to specific hypothalamic nuclei involved in body weight regulation. However, evidence is accumulating that, in addition to its pivotal role as an adiposity signal, leptin is a multi-faceted hormone that plays an important role in a plethora of CNS functions. In this review we summarize the recent advances made in leptin biology, with particular focus on its potential role as a cognitive enhancer and antiepileptic agent in the hippocampus.

Until recently, the secretion of gastric acid, which plays an important role in the pathogenesis of peptic ulcer, was thought to be controlled by diet, the autonomic nerves and gut hormones. However, peptic ulcer is now known to be caused by the infection with Helicobacter pylori (H. pylori), so it is possible that inflammation modifies the secretion of gastric acid. We used gastric-lumen-perfused rats to first examine the effect of interleukin-8 (IL-8) on acid secretion and then the involvement of free radicals and neutrophils in the action of IL-8. IL-8 enhanced tetragastrin-stimulated acid secretion and free radical scavengers or inhibitors and the pretreatment with anti-rat neutrophil serum inhibited this effect, which indicates that IL-8 enhances gastrin-stimulated acid secretion and that neutrophil-derived hydroxyl radicals mediate the IL-8-induced increase in acid secretion.

Synergism between genomic and non genomic estrogen action mechanisms.

The 17beta-estradiol (E2) action mechanism for inducing target gene expression can be attributed to both the direct binding of its receptor (ER) to specific sequences, the estrogen response element (ERE), and to the interaction between ER and other DNA-binding transcription factors. At the present meagre information is available for the role played by the rapid hormone action mechanism(s) (i.e., activation of ERK, PI(3)K, PKC-alpha) in modulating E2-induced gene promoter activity. Here the involvement of the rapid non genomic mechanism in triggering the transcriptional activity of two E2-regulated target genes in human hepatoma HepG2 cells has been studied, taking into consideration the cyclin D1 (ERE-devoid) and the complement protein 3 (ERE-containing) promoter-luciferase report constructs. The results indicate that the activation of the ERK pathway is essential for the E2-induced activity of both promoters, whereas the other rapid E2-induced membrane-starting signal transduction pathways (i.e., PI(3)K and PKC-alpha) were differently required. These results permit the amplification of the E2 action mechanism by the addition of synergy between the non genomic and genomic molecular actions of the E2-induced gene transcription.
