Intact PTH-producing hepatocellular carcinoma treated by transcatheter arterial embolization.

We report a case of hepatocellular carcinoma with hypercalcemia. There was no evidence of bone metastasis or increase in parathyroid hormone-related protein. The serum level of intact parathyroid hormone (intact PTH) was very high, and the results of the hepatic venous sampling suggested that the tumor produced intact PTH. Transcatheter arterial chemoembolization effectively controlled the humoral hypercalcemia.

Auxin and ethylene promote root hair elongation in Arabidopsis.

Genetic and physiological studies implicate the phytohormones auxin and ethylene in root hair development. To learn more about the role of these compounds, we have examined the root hair phenotype of a number of auxin- and ethylene-related mutants. In a previous study, Masucci and Schiefelbein (1996) showed that neither the auxin response mutations aux1 and axr1 nor the ethylene response mutations etr1 and ein2 have a significant effect on root hair initiation. In this study, we found that mutants deficient in either auxin or ethylene response have a pronounced effect on root hair length. Treatment of wild-type, axr1 and etr1 seedlings with the synthetic auxin, 2,4-D, or the ethylene precursor ACC, led to the development of longer root hairs than untreated seedlings. Furthermore, axr1 seedlings grown in the presence of ACC produce ectopic root hairs and an unusual pattern of long root hairs followed by regions that completely lack root hairs. These studies indicate that both auxin and ethylene are required for normal root hair elongation.

Phosphorylation of eukaryotic translation initiation factor-2alpha (eIF-2alpha) is one of the key steps where protein synthesis is regulated in response to changes in environmental conditions. The phosphorylation is carried out in part by three distinct eIF-2alpha kinases including mammalian double-stranded RNA-dependent eIF-2alpha kinase (PKR) and heme-regulated inhibitor kinase (HRI), and yeast GCN2. We report the identification and characterization of a related kinase, PEK, which shares common features with other eIF-2alpha kinases including phosphorylation of eIF-2alpha in vitro. We show that human PEK is regulated by different mechanisms than PKR or HRI. In contrast to PKR or HRI, which are dependent on autophosphorylation for their kinase activity, a point mutation that replaced the conserved Lys-614 with an alanine completely abolished the eIF-2alpha kinase activity, whereas the mutant PEK was still autophosphorylated when expressed in Sf-9 cells. Northern blot analysis indicates that PEK mRNA was predominantly expressed in pancreas, though low expression was also present in several tissues. Consistent with the high levels of mRNA in pancreas, the PEK protein was only detected in human pancreatic islets, and the kinase co-localized with somatostatin, a pancreatic delta cell-specific hormone. Thus PEK is believed to play an important role in regulating protein synthesis in the pancreatic islet, especially in islet delta cells.

Hyperinsulinemia induced by highly active antiretroviral therapy in an adolescent with polycystic ovary syndrome who was infected with human immunodeficiency virus.

OBJECTIVE: To report a case of polycystic ovary syndrome (PCOS) in an adolescent infected with human immunodeficiency virus (HIV) and lipodystrophy whose insulin resistance was induced by highly active antiretroviral therapy (HAART). DESIGN: Case report. SETTING: Academic department of pediatrics. PATIENT(S): A 14-year-old vertically HIV-infected adolescent receiving HAART. INTERVENTION(S): Clinical observation, metabolic and endocrinologic assessment, imaging of ovaries by ultrasound, and of intra-abdominal adipose tissue content by magnetic resonance imaging. MAIN OUTCOME MEASURE(S): Lipodystrophy, insulin resistance, PCOS. RESULT(S): After 34 months of HAART this adolescent showed lipodystrophy with central obesity and insulin resistance after oral glucose tolerance test (OGTT). Ovaries were normal at ultrasonography. After 56 weeks of HAART, the adolescent showed more severe evidence of lipodystrophy and insulin resistance, and she developed acne, hirsutism, and amenorrhea. The following hormone levels were elevated: FSH 5.9 mUI/mL, LH 15.4 mUI/mL, LH/FSH ratio >2.5, free T 5.6 pg/mL, DHEAS 2,070 ng/mL, androstenedione (A) 3.42 ng/mL; whereas 17-beta-E(2), P, PRL, and free T(4) values were within the normal range. Ultrasonography demonstrated a typical polycystic echographic architecture of the ovaries. CONCLUSION(S): Lipodystrophy and insulin resistance are well-recognized side effects of HAART. Polycystic ovary syndrome might be an additional side effect secondary to insulin resistance and lipodystrophy.

Hormonal influences on tryptophan binding to rat hepatic nuclei.

This study evaluated whether selected hormones, 3,5,3'-triiodothyronine (T3), hydrocortisone (HC), or insulin, would influence the binding of L-tryptophan to rat hepatic nuclei or nuclear envelopes. The first two hormones have nuclear receptors that belong to the same superfamily, while insulin belongs to a different unrelated superfamily of receptors. Previous reports have indicated that the binding of L-tryptophan to hepatic nuclear proteins was saturable, stereospecific, and of high affinity. Also, previous studies showed that administration of L-tryptophan rapidly stimulated hepatic protein synthesis. In this study, we investigated whether each hormone alone or together with unlabeled tryptophan would influence tryptophan binding to rat hepatic nuclei or nuclear envelopes as assayed by in vitro L-5-(3)H-tryptophan binding. Our results indicate that T3 10(-14) to 1(-10) mol/L appreciably inhibited in vitro 3H-tryptophan binding to hepatic nuclei and T3 10(-16) to 10(-4) mol/L appreciably ameliorated the inhibitory effect of unlabeled tryptophan (10(-4) mol/L) on in vitro 3H-tryptophan binding. In vivo administration (1 hour) of tryptophan alone stimulated hepatic protein synthesis, but addition of T3 negated such stimulation. Addition of HC 10(-12) to 10(-4) mol/L had no effect and addition of insulin 10(-16) to 10(-4) mol/L had only a small inhibitory effect on in vitro 3H-tryptophan binding to rat hepatic nuclei, but each (10(-12) to 10(-4) mol/L), when added to unlabeled tryptophan (10(-4) mol/L), diminished the inhibitory binding effect of unlabeled tryptophan alone. Our study indicates that T3 competes with tryptophan for hepatic nuclear tryptophan binding, and it also appears to negate tryptophan's stimulatory effect on hepatic protein synthesis.
