Breast feeding as seen by Islam.

PIP: Islamic law requires mothers to breast feed their children for 2 years. the father must assist the mother in breast feeding the children by providing her with food and clothing. If the father dies or does not live at home, the heir must support the mother thereby allowing her to breast feed her infants. A wet nurse can feed a child only if the mother's milk is not available. Allah recognizes that breast milk is the most wholesome food source for infants. It is made daily based on the infant's needs for growing and nourishment, while other milks and/or formulas remain the same regardless of the infant's daily requirements. For example, the 1st milk, a small amount of yellowish colostrum, contains protein, minerals, and antibodies. Colostrum also acts as a laxative. Breast milk is also comprised of enzymes which make breast milk easy to digest. These digestive enzymes also destroy any microorganisms in the stomach that may have entered thereby providing extra protection against disease. Further, breast milk is sterile. Infant mortality rates (IMRs) among breast fed infants are 4 time lower than IMRs of artificially fed infants. In addition, breast feeding establishes a strong bond between mother and child in which the child feels happiness and security. Breast feeding also benefits the mother. The uterus of a breast feeding mother fully contracts to its original size, while the uterus of a mother who does not breast feed does not. Further, breast feeding women are less prone to breast cancer than those women who have never breast fed. In addition, the more a women breast feeds the less likely she is to get breast cancer. Breast feeding also acts to a certain degree as a contraceptive.

Transitory laxative threshold of trehalose and lactulose in healthy women.

The transitory laxative threshold of a partially digestible disaccharide, trehalose, and an undigestible disaccharide, lactulose, was estimated by the dose-response relation between the test substance and the prevalence of diarrhea in 20 healthy female subjects. The subjects ingested several indicated amounts of trehalose or lactulose once daily 2 to 3 h after a meal. The intake of the test substance was stopped at the dose level that caused diarrhea or when the dose reached the maximal level. A record of physical conditions, gastrointestinal symptoms, and fecal conditions was made by all subjects before and after each ingestion of the test substance. Half the subjects experienced no diarrhea even with the ingestion of the maximal dose level (60 g) of trehalose in this study, and the ingestion of up to 40 g of lactulose caused diarrhea in 75% of all subjects. Abdominal symptoms such as flatus, distension, and borborygmus appeared at high prevalence with lactulose and trehalose ingestion, and the effect of lactulose was significantly stronger than that of trehalose at the same dose level (p < 0.05). The quantity of trehalose and lactulose that induced diarrhea differed greatly from person to person. The transitory laxative threshold was estimated as 0.65 g/kg body weight for trehalose and 0.26 g/kg body weight for lactulose by using the regression equation between the dose levels of the test substances and the cumulative incidence of diarrhea. These results suggest that it would be quite acceptable to administer trehalose up to 33 g and lactulose up to 13 g in a person weighing 50 kg.

BACKGROUND AND OBJECTIVE: Only a few studies have been done in Germany on the use of drugs in homes for the elderly and nursing homes. This is the first retrospective and longitudinal pharmaco-epidemiological study comparing the pattern of drug prescriptions for patients in nursing or for-the-elderly homes with that for health-insured ambulant patients or those not requiring care, all of them of the same age-group. PATIENTS AND METHODS: The samples were taken from a cohort of 3592 members of a company health insurance in Berlin, aged 60 years or over (912 men, 2680 women) who had been insured throughout 1999. The insurance company's database provided anonymous information on the insured and their doctors, details of drug prescriptions and hospital treatment. The patients were divided into three groups, group A: all those requiring care who lived in homes for the elderly or nursing homes (n=996); group B: all ambulant patients receiving care (n=1603), and group C: patients not requiring care, matched to the other groups for age and sex (n=993). All drugs were classified according to the anatomical-therapeutic-chemical WHO codes and listed by defined daily dosage (DDD). RESULTS: Those in group A received on average 3.6 DDD daily, those of group B 4.6 DDD, and those of group C 3.0 DDD. The relevant cost was 714 Euro (group A), 1126 Euro (group B) and 539 Euro (group C) per person in 1999. Indications of inadequate treatment concerned the use of psychopharmaceuticals (neuroleptics, antidepressives, hypnotics), analgesics, digitalis glycosides, antidiabetic drugs, diuretics, laxatives, and drugs of unproven efficacy. 15% of home residents received prescription from doctors on emergency call. These drugs were often continued beyond the immediate treatment. CONCLUSION: These data provide evidence of the lacking quality in the drug treatment of the elderly.
