The antibacterial activity of S-4661, a new parenteral carbapenem antibiotic, was assessed against the major urological pathogens isolated from patients with complicated urinary tract infections. S-4661 was slightly less active than imipenem and panipenem, but more active than meropenem and ceftazidime against Gram-positive bacteria. Against Gram-negative bacteria, S-4661 was similar to meropenem, similar to or more effective than imipenem, and more active than panipenem and ceftazidime. Thus S-4661 possesses potent and well-balanced wide-spectrum antibacterial activity against various urological pathogens.

Evaluating the adherence factor of uropathogenic Escherichia coli is important for assessing the relative efficiency of antimicrobials when used at sub-minimal inhibitory concentration (sub-MICs). The microdilution method was used to determine the MICs of gentamicin, trimethoprim-sulfamethaxozole and penicillin. Then the efficacy of antimicrobial sub-MICs was determined by hemagglutination and adherence assays. Instead of showing nearly the same MICs, gentamicin had nearly twice the activity of trimethoprim-sulfamethaxozole. Gentamicin, as a "long acting" agent, can be accepted as being more effective than trimethoprim-sulfamethaxozole or penicillin, especially at sub-MICs, against adherence factors of uropathogenic E. coli, and can be used as monotherapy for urinary tract infections.

According to economic principles an inappropriate prescription is the choice of an antimicrobial with higher/equivalent cost and lower effectiveness (or higher cost and equivalent/lower efficacy) than an alternative (in this case, the former is specified as a "dominated" drug). To identify cost-effective antibiotics we applied the principles of incremental cost-effectiveness analysis (ICEA) to microbiological data of San Bortolo Hospital. Its 27 wards were grouped in 9 functional areas. The resistance patterns of 8 urinary pathogens in the 1997 microbiology data base were assessed. The measure of antibiotic effectiveness was expressed as the percentage of isolates susceptible to each antibiotic tested. The difference in cost (i.e. the incremental change) between each antibiotic and the next more expensive alternative was calculated, and compared with the incremental change in effectiveness. Calculations were made for each pathogen. The antibiotics remaining after exclusion of all "dominated" antibiotics were pooled on a list defined as "Specific Area Formulary". The implications of the use of economic principles within a general antimicrobial policy are discussed.

Impedance characterization of a piezoelectric immunosensor part II: Salmonella typhimurium detection using magnetic enhancement.

This study investigated the usefulness and characteristics of a 5-MHz quartz crystal resonator as a sensor of biological pathogens such as Salmonella typhimurium. An impedance analyzer measured the impedance behavior of the oscillating quartz crystal exposed to various concentrations of Salmonella (10(2)-10(8) cells per ml). The Salmonella cells were captured by antibody-coated paramagnetic microspheres, and then these complexes were moved magnetically to the sensing quartz and were captured by antibodies immobilized on the crystal surface. The response of the crystal was expressed in terms of equivalent circuit parameters. The motional inductance and the motional resistance increased as a function of the concentration of Salmonella. The viscous damping was the main contributor to the resistance and the inductance in a liquid environment. The load resistance was the most effective and sensitive circuit parameter. A magnetic force was a useful method to collect the complexes of Salmonella-microspheres on the crystal surface and enhance the response of the sensor. In this system, the detection limit, based on resistance monitoring, was about 10(3) cells per ml.

According to the WHO definition the term diabetes mellitus covers a group of metabolic diseases characterized by elevated blood glucose levels. The two main representatives of this group are type 1 and type 2 diabetes. All forms of diabetes have in common that, due to hyperglycemia, complications (vascular damage in particular) can occur, which are responsible for the high rate of morbidity as well as the excess mortality of the group of persons affected. In all populations--Germany included--the overwhelming majority of diabetics are subjects with type 2 diabetes. Type 2 diabetes is almost 20 times more common than type 1 diabetes, i.e. almost 90% of all diabetic persons can be assigned to the type 2 diabetes category. The rest can be split up into 5-7% type 1 diabetics and 2%-5% of persons suffering from secondary forms of diabetes (pancreatic and hepatic diseases). Positive antibody tests in older diabetic persons suggest that type 2 diabetes may not be a uniform disease and that in almost 10% of cases an autoimmune disease might be the cause of diabetes in old age (LA-DA diabetes). The data of the diabetes register of the former German Democratic Republic (East Germany) together with representative samples obtained from the federal states that comprised West Germany indicate a diabetes prevalence of about 5% in the adult population of Germany. Once the new WHO diagnostic criteria for diabetes mellitus are used and an early-diagnosis procedure (the oral glucose tolerance test) is widely applied the prevalence is expected to be shown to be higher. What is more, the global trend toward higher figures for type 1 and type 2 diabetes can also be observed in Germany. Whereas the pathogenesis of type 1 diabetes results from the autoimmune destruction of beta cells, leading to insulin deficiency, in type 2 diabetes insulin resistance as well as impaired insulin secretion are present; mechanisms that interact closely in the development of glucose intolerance. Strong genetic and environmental factors operate with regard to both types. The reduction of the life expectancies for type 1 and type 2 diabetes is contingent upon the appearance of micro- and/or macrovascular complications. At the top of the mortality statistics are patients suffering from cardiovascular and/or kidney diseases. However, by applying metabolic control and by means of other intervention strategies the prognoses for diabetic patients can be improved significantly.
