Implementing assertive community care for patients with schizophrenia. A case study of co-operation and collaboration between mental health care and social services.

The aim of this study was to describe the personal experiences of collaboration and co-operation between staff from the community mental health and social services programmes in their daily work with schizophrenic patients. An additional aim was to generate the first step in a theory of how staff in the field of health care and social services manages to integrate their efforts on an individual and organizational level. The study group consisted of five members of a multidisciplinary team who had been working for 2 years with assertive outreach intensive clinical case management. Verbatim-transcribed thematic interviews were analysed according to the comparative method for grounded theory. The analysis was focused on quality of interpersonal interaction between practitioners, representing different professions in community-based mental health care, as well as the professional relationship to the patients/clients and their relatives. In the light of an individual and organizational perspective, different typologies of interpersonal interactions appeared. Three main variables were identified: collaborative relationship, co-operative partnership and professional-amateurism. Our findings on these qualitative differences in interpersonal relations should be taken into account in the continuing reorganization of the community health care services. This knowledge can also help in overcoming barriers between patients, professionals and organizations in the mental health services.

Taxonicity of adolescent melancholia: a categorical or dimensional construct?

A taxometric analysis was conducted to test the hypothesis that the latent structure of melancholia in adolescents is categorical. Two taxometric procedures were used: Mean Above Minus Below a Cut (MAMBAC) and Maximum Covariance (MAXCOV) analyses. Participants were 378 adolescents presenting for a depression evaluation. Indicators of melancholia were constructed using items from the Schedule for Affective Disorders and Schizophrenia for School Aged Children (K-SADS) and the Beck Depression Inventory (BDI). The indicators of melancholia were consistent with a categorical latent variable. The findings suggest that the latent structure of melancholia in adolescents is similar to its previously identified categorical structure in adults. Implications for clinical research are discussed. 2002 Elsevier Science Ltd.

Reduced prepulse inhibition in rats with entorhinal cortex lesions.

The relationship between the entorhinal cortex and prepulse inhibition (PPI) as well as dopaminergic participation in this relationship were examined. PPI is an operational measure of sensorimotor gating in which a robust response to a startling auditory pulse stimulus is inhibited when the stimulus is preceded by a weak prepulse. PPI can be measured in various species and is reduced in several neuropsychiatric disorders and in dopamine-activated rats. The entorhinal cortex was damaged bilaterally using ibotenic acid, and acoustic startle experiments were performed during treatment with haloperidol or saline on day 21 after the ibotenic acid injection. Neither this injection nor haloperidol affected the amplitude of the startle movement. Bilateral entorhinal cortex lesions reduced PPI, while haloperidol partially restored it. The entorhinal cortex and the sensorimotor gating system therefore may be related via dopaminergic circuits, possibly including the nucleus accumbens. Further, as the entorhinal cortex provides the major extrinsic synaptic input to the rat hippocampus, disease involvement of this region may severely affect cognition in various disorders including schizophrenia. 2002 Elsevier Science B.V.

Weight gain is a well-known complication of antipsychotic therapy, especially when using newer atypical antipsychotic agents. In addition to the risk of medical comorbidities associated with weight gain, such as diabetes mellitus and cardiovascular disease, a further risk of antipsychotic-induced weight gain is that patients may resort to over-the-counter preparations to aid in weight loss. We report on a case in which a patient with schizophrenia began using Metabolife, an herbal preparation containing ephedra, for weight reduction and subsequently developed an exacerbation of his psychosis with superimposed delirium. We mention several relatively safe alternatives to herbal supplements for weight loss, as well as emphasize the need for clinicians to educate their patients regarding the potential risks of over-the-counter weight loss agents.
