17Beta-estradiol rapid stimulation of rat aorta NOS activity is prevented by oestrogen deficiency.

OBJECTIVES: The purpose of this study was to determine the effects of chronic oestrogen deficiency on rat aorta rapid response to 17beta-estradiol treatment. METHODS: Rat aortic strips (RAS) were isolated from Wistar female rats of three different groups: rats 6-7-month old with normal oestrogen levels (NER); aged rats, 24-month old, with low oestrogen levels (LER); and young rats after 2 months of bilateral ovariectomy (OVX). Platelet aggregation was measured after incubation of RAS in a platelet rich plasma by addition of 10 microM ADP. NO production by RAS was measured by 3H-citrulline technique. RESULTS: RAS obtained from NER treated with 17beta-estradiol produced an inhibition of platelet aggregation specific for ovarian hormones, since testosterone was devoid of any effect. In aortic tissue isolated from male rats no increment in nitric oxide (NO) production was found. RAS from LER and OVX treated with 1-10 nM failed to induce a significant inhibition of platelet aggregation compared with NER (5 and 17%; 6 and 20% vs. 45 and 77% inhibition of platelet aggregation respect to control, respectively). In contrast to NER, 5 min treatment of LER and OVX aortic tissue with 1 nM 17beta-estradiol did not incremented NO production (NER 1.14 vs. 2.3 (P < 0.05); LER 1.14 vs. 1.42; OVX 1.24 vs. 1.52 pmol NO per mg protein). CONCLUSIONS: These results suggest that chronic oestrogen deprivation impairs the inhibition of platelet aggregation and suppresses the rapid stimulation of aortic NOS induced by acute 'in vitro' treatment with 17beta-estradiol. 2002 Elsevier Science Ireland Ltd.

Gene-drug interaction: additive influence of mutant APOA1 and testosterone on plasma HDL-cholesterol.

OBJECTIVES: Factors associated with decreased plasma high-density lipoprotein (HDL) cholesterol concentration, or hypoalphalipoproteinemia, include androgenic steroids and mutations in APOA1, encoding apolipoprotein (apo) A-I, the main structural protein of HDL. However, there is little information regarding the extent of plasma HDL lowering when exogenous testosterone is used in subjects with monogenic low HDL. DESIGN AND METHODS: A man with coronary heart disease (CHD) had been receiving exogenous testosterone post-orchidectomy. He had marked hypoalphalipoproteinemia, which was not responsive to diet or medication. To identify a possible genetic contribution to his biochemical phenotype, we sequenced the LCAT and APOA1 genes. RESULTS: There were no sequence abnormalities in LCAT, but we found that he was a heterozygote for a novel APOA1 mutation in codon 107 (AAG->TGG), which predicted the replacement of lysine by tryptophan (K107W). Serial biochemical measurements over 11 years showed that plasma HDL cholesterol on either intramuscular or oral testosterone was 0.19 +/- 0.06 mmol/L, while plasma HDL cholesterol on transdermal testosterone was significantly higher at 0.52 +/- 0.18 mmol/L (p = 0.015, unpaired t-test). CONCLUSIONS: The findings suggest that the low plasma HDL cholesterol associated with heterozygosity for mutant APOA1 can become extremely depressed during treatment with oral or intramuscular androgens. The findings also suggest that transdermal testosterone may perturb HDL to a lesser extent than other routes of delivery in such patients.

PIP: The development of the synthesis of potent orally active 19-norsteroids and the beginning of the use of steroid hormones for contraception via ovulation suppression is reviewed. Chemical structures are shown for 19-nortestosterone progestogens: removal of the methyl group from position 19 increased the potency and additional potency was obtained by addition of an acetate group at positions 3 and 17 or of cyclopentylenol ether at position 3. 19-nortestosterone derivatives are partly metaboloized to estrogens; therefore they are called estrogenic testosterone. However they have no detectable estrogenic action. Chlomadinone is the most potent substituted progestogen and ethynodiol acetate is the most effective 19-norsteroid. Clinical applications are: 1)proved indications--dysfunctional uterine bleeding, menorrhagia, hypomenorrhea, amenorrhea, primary dysmenorrhea, endometriosis, metastatic endometrial carcinonma, and contraception; 2)proposed indications--precocious puberty, increase in linear growth in girls; timing of menstruation hirsutism of ovarian origin, idiopathic infertility and inadequate luteal phase, habitual abortion, and metastatic breast carcinoma.

Long-term administration of norethindrone in fertility control.

PIP: Fertility control by cyclic norethindrone (Norlutin), 17 alpha-ethinyl 19-nortestosterone, plus .06 mg 3-methoxy ethinyl estradiol (Ortho-Novum) was studied in 364 women over a period of 32 months for a total of 6062 cycles. No patient who followed the instructions became pregnant. 37 patients stopped the medication for various reasons. The interval between stopping medication and becoming pregnant averaged 1.6 months. 13 of these pregnancies occurred after 11-15 cycles of treatment. Children born to these mothers were normal with no virilization observed. Findings from all Papanicolaou smears and cervical biopsies were normal. The desirable effects of diminishing the menstrual flow, reducing dysmenorrhea and regulating the menstrual cycle, plus the all-important one of contraception, far outweighed minimal and infrequent undesirable side effects (in order of frequence: chloasma, hot flashes, headache, nausea, acne, abdominal pain, dizziness and urticaria). In only 4.8% of the total 6062 cycles was some complaint made.
