Chromosomal aberrations and micronuclei in lymphocytes of patients before and after exposure to anti-tuberculosis drugs.

To evaluate the genetic damage induced by tuberculosis infection and also by a combination of therapeutic exposure to anti-tuberculosis (anti-TB) drugs (isoniazid + rifampicin + pyrazinamide + ethambutol or streptomycin) we have considered chromosome aberrations (CA) and micronuclei (MN) in binucleate peripheral blood lymphocytes from 36 patients diagnosed with pulmonary tuberculosis prior to receiving anti-TB drugs in the first phase and in the second phase after 6 months of therapy with anti-TB drugs. The same cytogenetic study has also been performed on 36 healthy individual controls. It was shown that the frequencies of both CA and micronucleated binucleate cells increased significantly after therapy with anti-TB drugs as compared with TB patients and controls. In addition, we observed that both cytogenetic markers (CA and MN) in patients before treatment with anti-TB drugs were significantly (P: < 0.05) higher than controls. No relationship was found between the frequency of MN and donor age. Thus, apart from drug effects, infection with tuberculosis is associated with increased CA and MN.

Pulmonary tuberculosis in pregnancy.

Tuberculosis (TB) was once a formidable public health hazard but is now less feared because of the development of an armamentarium of effective drugs. The incidence of TB had been declining for decades until recently when the number of cases of TB began to rise. The most obvious reasons for this trend were the rising rate of human immunodeficiency virus infection and the development of multiple drug-resistant TB. The largest percentage increase in TB cases occurred among persons 25 to 44 years of age; many of them are women. A number of these women will first be diagnosed with TB infection during pregnancy. It is important for the clinician to be aware of the impact of TB on this population. Patients must be screened as recommended by the Centers for Disease Control and Prevention because it is important to quickly diagnose TB. Prompt and effective therapy will benefit the mother and the neonate. This article reviews the diagnosis, prevention, and treatment regimens of TB in pregnancy, as well as the effects on the HIV patient, the neonate, and breastfeeding. Prompt recognition and treatment of TB infection will ensure that the rates of TB cases decline.

After a brief reminder concerning the PET technique and use of FDG, the author sets out to summarize the results of numerous PET centers in the world and in more detail, results of European units he visited. Leaving neurology and cardiology out, the most important works are dealing with tumoral pathology. The sensitivity of the technique is high for localizing tumoral tissue, evaluating lymphatic spread and detecting metastases. On the contrary, specificity is low as non tumoral lesions, especially inflammatory lesions, tuberculosis may pick up FDG though with less intensity. PET contributes to precise staging TNM of cancer, especially in case of pulmonary neoplasms. Results of clinical evaluation of bronchial carcinoma are presented. Other research programs show similar positive results for the staging of digestive tract neoplasm, primary or metastatic lymph node diseases, metastases from a quiescent, unknown primary tumor. The fact that the number of PET units is small explains the relatively poor dissemination of the method. It is rather obvious that the limitation of the number of PET units is due to economic reasons. Since 1990, many scientific evaluations of this technique conclude positively on PET's future. As it essentially is a functional method, the future developments of PET will largely depend on the discovery of new tracers.

[The morphological reactions in the lungs during the chemotherapy of experimental tuberculosis]

Histological, histochemical, and electron microscopic studies were used to examine cellular and subcellular responses in the lung to the development of tuberculous inflammation and its drug treatment. In early inflammation, they are induced by higher capillary and cellular permeabilities, cell ultrastructural changes in the air-blood barrier. The period of granuloma formation is characterized by a larger count of hypertrophic alveolar parenchymal cells, type 2 alveolocytes in particular, that synthesize a surfactant, and by higher macrophagal activity. During therapy, there are increases in the count of multinuclear giant cells in the granuloma, in the resolution of inflammatory changes or in the isolation of a tuberculous focus. Morphological signs that show the inefficiency of treatment are identified.

[The monitoring of the drug resistance of the causative agent of tuberculosis in Russia in 1979-1998]

The Russian 1979-1998 annual reports of the centralized microbiology laboratories of the tuberculosis controlling service were used to examine the pattern and trends of formation of drug-resistance in the tuberculosis pathogen. The data on over 1.2 million cases of drug resistance to 7 antituberculous drugs were pooled. The analysis indicated that drug resistance had increased in two steps in the past 2 decades. The maximum levels of the drug resistance of Mycobacterium tuberculosis to some drugs were limited (7.8 and 37.2%), which are not greater those as indicated by the international statistics.
