[The efficacy of the ambulatory treatment of patients with newly detected pulmonary tuberculosis]

Scientific rationale has been provided for the fact that new cases of pulmonary tuberculosis can be successfully treated in the outpatient setting. The paper suggests that the pattern of a tuberculous process and the patient's socioeconomic status and education level should be taken into account while choosing a treatment regimen. Indications, contraindications, necessary conditions for outpatient treatment are defined. General recommendations how to use an antibacterial treatment regimen and its methods are given. The efficiency of outpatient and inpatient treatment regimens is compared in the matched groups of patients and the advantages of treatment in the outpatient setting are noted. Treatment in the outpatient setting shows a 2-fold decrease in the likelihood of preschedule discontinuation of the basic course of antibacterial therapy as compared to that in a 24-o'clock hospital, a 2.4-fold reduction in the length of temporary disability and it can also decrease the basic course of antibacterial therapy by 1.5-2.5 months. With this, its therapeutical efficiency does not drop.

[Impact of Yersinia pseudotuberculosis on the in vitro production of cytokines by whole blood cells of donors]

The impact of two plasmid (47, 82 MD), single plasmid (47 MD) and non plasmid Y. pseudotuberculosis strains, Y. enterocolitica (47 MD) as well as Y. pseudotuberculosis superantigen (YPM) on the production of interleukin-1 (IL-1), interleukin-6 (IL-6), interferon-alpha (IFN = alpha) and tumor necrosis factor alpha (TNF-alpha) by whole blood cells obtained from donors was studied. All Y. pseudotuberculosis and Y. enterocolitica strains stimulated the production of IFN-alpha, IL-1, IL-6 and TNF-alpha by whole blood cells, but considerably less than Y. pseudotuberculosis lipopolysaccharide and YPM. These data are indicative of the pathogenetic role played by 82 MD plasmid in manifestation of Y. pseudotuberculosis immunosuppressive properties. The maximum stimulation of the production of cytokines was observed under the action of YPM, which confirmed an important role played by this superantigen in the pathogenesis of Y. pseudotuberculosis.

[The diagnosis and treatment of spontaneous pneumothorax of different etiologies]

A total of 1210 patients with spontaneous pneumothorax (SP) and spontaneous pyopneumothorax (SPP)' were treated at the Surgery Unit of the Santariskes Tuberculosis and Lung Diseases Hospital in 1960-1997. Of them, 533 (45.7%) patients had the conditions of tuberculous origin and 657 (54.3%) had nonspecific SP. Treatment with punctures was indicated for closed SP and degrees I and II collapse. If it was ineffective, the authors applied drainage 4-5 days later. The indications for this treatment were ineffective punctures, open and tension SP, pleural fluid, and SPP. If following 5-6 days of aspiration, the lung did not restore, surgery was made. A total of 1108 (91.6%) patients fully recovered and 102 (8.4%) died.

[The surgical treatment of destructive forms of tuberculosis of the cervical spine]

The paper analyzes the outcomes of surgical treatment in 19 patients with tuberculosis of the cervical part of the vertebral column. All the patients had surgical treatment: abscessotomy, sparing resection of a destruction area, decompression of the spinal cord, removal of kyphotic deformity by restoring the axis of the vertebral column. The existing defect was filled with an autograft in 7 cases, with a biocompatible implant by applying benemycin to its surface in 12 cases. Posterior fixation of the diseases part of the spinal apparatus was made in 8 cases. Good outcomes were followed up 8 months to 7 years after surgery. At X-ray study, the bony block formed between the resected vertebral bodies within 3 to 7 months. Neurological symptoms were arrested in all cases, except 4 ones. One patient was found to have a slight lower extremity weakness and 3 patients had correctable radicular symptoms.

The law, human rights, and the detention of individuals with tuberculosis in England and Wales.

BACKGROUND: Tuberculosis poses a global public health threat, and individuals who fail to comply with treatment risk developing drug-resistant strains, which are a serious public health concern. A number of individuals who have been deemed to pose a 'serious risk of infection' to others have been detained in recent years in England and Wales under the Public Health Act 1984. With the incorporation of the European Convention on Human Rights (ECHR) into British law due to take effect shortly this paper examines the justness of Sections 37 and 38 of the Act, and asks whether the Act stands up to scrutiny under the ECHR. METHODS: A critical review, including an examination of recently opened relevant files at the Public Record Office, was carried out on Sections 37 and 38 of the Public Health Act 1984. RESULTS: Sections 37 and 38 of the Public Health Act 1984 fail to provide sufficient safeguards from abuse and fall short of the requirements of the ECHR. CONCLUSIONS: Sections 37 and 38 should be replaced. Greater safeguards to protect the rights of those with infectious diseases are needed.
