Assessment of the impact of BCG vaccination on tuberculosis incidence in south Asian adult immigrants.

Although BCG vaccination is recommended for tuberculin negative adult immigrants, there has hitherto been no evidence of its efficacy in this group. This epidemiological study compares the incidence of tuberculosis in a cohort of South Asian adult immigrants vaccinated on entry to the UK with its incidence in cohorts of immigrants of the same age and origin estimated from the five-yearly national surveys. Results suggest that BCG vaccine may have reduced the incidence of tuberculosis by up to 87.6% (95% confidence interval 55.2% to 98.5%) and support the current recommendation to vaccinate this high-risk group.

The aim of this retrospective study was to evaluate the yield of bronchial endoscopy for the diagnosis of tuberculous mediastinal adenopathies. We analyzed the results in 200 procedures in patients with tuberculous mediastinal adenopathies. Mediastinal tuberculous adenopathies were found in 6% of the patients with tuberculosis diagnosed during the same period. Mean age of the patients was 30.5 years, and the sex ratio was 1.5. Bronchial endoscopy improved the diagnostic yield of the bronchial aspiration fluid (22% versus 11% without endoscopy). This method allowed positive diagnosis of tuberculosis in 164 patients (82%) by visualizing specific lesions such as compressions, granulations, and gangliobronchial fistulae. Histological confirmation was obtained in 72 (79%) patients among 91 biopsy specimens. Bronchial endoscopy remains a crucial exploration for the diagnosis of mediastinal tuberculous adenopathy, even in HIV-infected patients, because in our series, among 101 treated patients, 99 were HIV positive (98%).

The epidemic form of Kaposi's disease, called Kaposi-AIDS, was first described in 1981 by Hymmes in two young AIDS patients. Lung lesions are observed in 10 to 45% of patients with cutaneous Kaposi and are the second leading localization of visceral involvement after the digestive tract. Diagnosis and management of these visceral forms remains difficult. We report two cases of broncho-pulmonary Kaposi-AIDS in two patients treated for cutaneous Kaposi's disease. The diagnosis was made on the basis of epidemiological, clinical, biological, radiological, endoscopic, and histological evidence. The diagnosis was achieved by elimination after ruling out all other opportunistic diseases with pulmonary tropism. The fear of miliary tuberculosis remains strong in zones with a high prevalence of tuberculosis. Despite considerable improvement with antiretroviral drugs, especially antiproteases, outcome remains unpredictable with or without specific treatment.

Variation of the milk antibody response to paratuberculosis in naturally infected dairy cows.

A longitudinal study was performed to determine the course of the milk antibody response in cows presumably infected with Mycobacterium avium subsp. paratuberculosis. Milk samples were collected repeatedly (1 to 10 times) from all lactating cows in seven Danish dairy herds. A total of 4,289 observations from 812 cows was analyzed after exclusion of samples collected after 280 days in milk (DIM). The level of antibodies in the milk samples was assessed using an indirect ELISA. A piece-wise linear random coefficient regression model was specified. The model controlled for the effect of herd, breed, laboratory effects, and age at first calving to estimate parity-specific antibody responses in relation to DIM. Separate antibody profiles were estimated for fecal culture-positive and fecal culture-negative cows. The resulting population average models showed higher antibody levels for fecal culture-positive cows and higher antibody levels with increasing parity. On average, the antibody response was high at the beginning and end of lactation. However, evaluating the cows individually indicated that most cows actually had quite stable ELISA levels throughout lactation, with some cows having higher levels than others. Thus, two criteria seem applicable to assess whether a cow is infected: stability and ELISA level. The random coefficients for each cow were highly significant. Thus, the study suggests that all cows can be classified into one of the four categories by combining the cow-level ELISA characteristics "stability" and "level" as an aid in the diagnosis ofparatuberculosis and thereby substantially increasing the sensitivity of the ELISA.

Prospective study of coroner's autopsies in Benin City, Nigeria.

A prospective study of 726 cases of coroner's autopsies carried out over a period of two years (1st January, 1996 - 31st December, 1997) was reviewed. The causes and circumstances of death are defined and classified into natural and unnatural deaths. Fifty cases (6.3%) were children while the remaining 676 cases (93.1%) were adults in a male to female ratio of 2.6 to 1. The most common indications for coroner's autopsies were sudden unnatural deaths (SUDs) accounting for 485 cases (66.8%) and also sudden unexpected natural deaths (SUNDs) (241 cases) accounting for 33.2%. In the first category, road traffic accidents accounted for 86.7% of cases with deaths involving motor vehicle drivers and their passengers (41.8%); pedestrians (37.1%); and motorcyclists and their passengers (18.6%). The commonest causes of death in all road traffic accidents were craniocerebral injuries and haemorrhagic shock. In the second category the most common causes of sudden unexpected natural death were cardiovascular diseases resulting from complications of hypertension (54.7%) occurring in apparently healthy individuals. Other causes of death were pneumonia, pulmonary tuberculosis, typhoid fever and neoplastic diseases. This study showed that in Nigeria, with an increasing acquisition of dietary and life style habits of the developed western world, there is becoming a concomitant risk of deaths from road traffic accidents and sudden unexpected natural deaths from hypertensive cardiovascular disease.
